The states requires all 6th graders to have a documented physical. Please take a physical form to your child's next well visit for the office to complete it, and submit the completed form, along with a copy of the immunizations, to the NURSE or MAIN OFFICE (who will see that the nurse receives it). PLEASE remember..... the Tdap and MCV (meningitis) vaccines are due for the START of 7th grade. Please discuss this with your child's physician at their 6th grade well visit, and be sure to send in a printout of any immunizations they receive for their school record.*Scoliosis screenings are mandated by the state in 6th grade. This screening will take place in the phys ed class in the spring (April) IF the nurse has not received a physical form by that time which has been completed by the physician indicating that this was checked at the well visit.
